CHILDREN’S SERVICES OUTPATIENT CASE MANAGER: We are seeking a qualified professional to provide comprehensive services and support for children and youth with primary mental health diagnoses. This role focuses on skill development, community participation, and fostering independence in home, school, and community settings through structured interventions and collaborative care. The goal of our Children’s Services Case Manager is to implement evidence-based strategies that support children and youth in achieving daily living skills, social integration, and functional independence. The successful candidate will coordinate with families, educators, and community partners to ensure continuity of care, promote adaptive behaviors, and enhance overall well-being. This role requires expertise in mental health support, case management, and program implementation to effectively meet the needs of individuals in a structured, goal-oriented manner.
PAY RANGE/BENEFIT PACKAGE:
Salary: starting at $55,181.18 annually - placement above minimum salary is based on experience.

- Bachelor’s degree in social work, psychology, human services, child and family studies or related field.
- 1 year experience working with children, youth, and families.
- Community-based service delivery experience preferred.
- Previous experience working with mental health diagnoses/treatment related to children, youth and families.
- Commitment to person and family centered service planning and delivery.
- Ability to work independently within a strong interdisciplinary team.
- QIDP, QMHP, or CMHP preferred.
- Facilitate the person-centered planning process.
- Monitor services including assessments, planning, and linking.
- Advocate for the child and serve as a link between schools, healthcare providers, and community agencies.
- Support children and their families to help them live successfully in the community/least restrictive environment.
- Manage information within a multidisciplinary team.
- Work collaboratively with the Children’s Outpatient Therapists to ensure goals and plans of service are up to date
- Work collaboratively with care coordinators to ensure all internal and external coordination and processes are implemented.
- Case documentation and assessments
- Travel required with reimbursement.
- Ability to work well in various settings.
- Establish and maintain good relationships with consumers and community professionals.
- Flexible hours based on the needs of children and their families.
- Utilize strength-based approaches.
- Performs other duties as assigned.
