Recovery Management Community Based Clinician: OnPoint is seeking an energetic, organized, committed professional to fill the position of full-time Recovery Management Community Based Clinician. As a Recovery Management Clinician, you will be responsible for providing outreach, therapy and clinical case management for individuals who are living with substance use, mental health or co-occurring disorders who require a higher level of care than can be provided through traditional outpatient model.
PAY RANGE/BENEFIT PACKAGE:
Salary: starting at $63,876.80 up to $88,171.20 annually - placement above minimum salary is based on experience.

REQUIRED QUALIFICATIONS:
Master’s degree in a discipline that supports public- and third-party reimbursement, from an accredited college or university.
State of Michigan professional license or certification at the master’s level.
Minimum of one year of relevant professional experience working with individuals with substance use or co-occurring disorders
CAADC-DP or willingness to pursue within one month of hire, if not currently a LMSW or LPC
PREFERRED QUALIFICATIONS:
One year’s experience as a master’s level clinician.
CAADC or LMSW or LPC
Public behavioral health system experience.
Certification in relevant evidence-based practices
At least one year’s experience working with serious mental illness or co-occurring disorders
Lived experiences with mental illness/developmental disabilities/co-occurring disorders/substance use disorders.
ADDITIONAL REQUIREMENTS:
Must possess a valid driver’s license, registration and proof of automobile insurance when operating County vehicles or operating personal vehicles in performance of job duties at any time. May provide own transportation for job related use, including transporting of individuals served, to and from meetings and activities at varying work locations throughout Allegan County.
GENERAL RESPONSIBILITIES:
Engage and support target population through community-based outreach, and therapy according to accreditation, funding, regulatory and best practice standards.
Outreach includes in-person and phone contacts and follow-up to identify needs and resolve barriers to engagement.
Clinical therapy focuses on social determinants of health while incorporating clinical assessment and best practice.
Treatment includes individual, group and family therapy for substance use and co-occurring disorders, utilizing a recovery-oriented system of care framework.
Screening and assessment for substance use and mental health disorders, plus intellectual and developmental disabilities.
Develop measurable person-centered plans of service in partnership with individuals being served and their teams, assuring a trauma-informed and strength-based approach.
Utilize evidence-based practices in provision of individual and group therapy.
Partner with peer recovery coaches, case managers, care coordinators, and med clinic staff to offer a unique team approach that addresses recovery goals in the context of social determinants of health and level of care determination.
Document services according to established standards.
Participate in utilization management and quality improvement processes.
Actively engage in supervision and team meetings.
Engage in internal and external care coordination efforts, including interdisciplinary team meetings.
Perform other duties as assigned.
