HOUSING CASE MANAGER: OnPoint is seeking a dedicated and compassionate individual to join our team as a full-time Housing Case Manager. In this vital role, you will support individuals and families in Allegan County who are experiencing homelessness or are at imminent risk of losing their housing. The Housing Case Manager is responsible for assessing client needs, developing individualized housing plans, and connecting clients with resources and support services to secure and maintain safe, stable housing. This position requires strong organizational skills, a collaborative spirit, and a commitment to empowering vulnerable populations through client-centered advocacy and solution-focused case management.

PAY RANGE/BENEFIT PACKAGE: 
Salary: starting at $55,181.18 annually - placement above minimum salary is based on experience.

REQUIRED QUALIFICATIONS:
  • Bachelor’s degree in social work, psychology, human services, or related field.
  • One year experience working with adults diagnosed with an intellectual /developmental disability or mental health disorder.
PREFREERED QUALIFICATIONS:
    • Community-based service delivery
    • Knowledge of person-centered planning process
    • One year of professional experience in homeless services
    • Lived experiences with mental illness/developmental disabilities/co-occurring disorders/substance use disorders.  
GENERAL RESPONSIBILITIES:
  • Deliver services according to funding, regulatory, and best practice standards.
  • Orient clients to housing programs for which they are eligible.
  • Maintain working knowledge of community resources and available rental housing units.
  • Develop and maintain effective working relationships with landlords and property managers.
  • Actively engage clients in development and implementation of an individualized service plan
  • Facilitate access to available financial subsidies and concrete supports.
  • Internal and external case collaboration and coordination, including gathering and exchanging relevant information.
  • Documentation of services and supports in accordance with funding, regulatory, accreditation and best practice standards.
  • Support individuals on assigned caseload through advocacy, monitoring, referring, and linking with other providers.
  • Participate in supervision, team, and other identified meetings.
  • Perform other duties as assigned.

POSITIONS TO BE FILLED: 
  • One full time positions (40 hours/week) 
This is a grant funded position with potential for sustainability within the Housing Program
