MEDICATION CLINIC CASE MANAGER: The Medication Clinic Case Manager is responsible for providing comprehensive case management services to individuals engaged with the medical clinic, primarily those receiving psychiatric medication management. This role involves the development and coordination of Individual Plans of Service (IPOS) for clients who primarily receive psychiatric medication services without additional mental health treatment. The Case Manager will work closely with clients to assess their needs, create personalized care plans, and ensure that clients are connected to appropriate services. Additionally, the Case Manager plays a crucial role in assisting clients in establishing and maintaining relationships with primary care providers to ensure continuity of care and support their overall health and well-being.  

PAY RANGE/BENEFIT PACKAGE:  

Salary Range: starting at $55,181.18 up to $76,150.02 annually - placement above minimum salary is based on experience.

REQUIRED QUALIFICATIONS: 

  1. Bachelor’s degree in social work (BSW), Psychology, Counseling, or a related field. 

  1. A minimum of 1-year prior experience in behavioral health case management, medical social work, care coordination, or related experience. 

  1. Lived experiences with mental illness/developmental disabilities/co-occurring disorders/substance use disorders valued.   

PREFERRED QUALIFICATIONS: 

  1. Licensed/Limited Licensed Bachelor of social work (LLBSW/LBSW) 

  1. Qualified Intellectual Disabilities Professional (QIDP) 

  1. Qualified Mental Health Professional (QMHP) 

  1. Children’s Mental Health Professional (CMHP) 

  1. Certified Alcohol and Drug Counselor (CADC) 

GENERAL RESPONSIBILITIES: 

  • Facilitate the person-centered planning process. 
  • Provide Case Management Services to individuals on assigned case load through: 
    • Transportation as required by treatment plan. 
    • Development of Natural Supports including family, friends, religious organizations/leaders, neighbors, teachers, employers, and all others involved and interested in supporting the consumer. 
    • Internal and external case collaboration and coordination, including gathering and exchanging relevant information. 
    • Participation in clinical case conferences, staff development (in-services, conferences, professional organizations) and agency committees as assigned. 
    • Support individuals on assigned caseload through advocacy, monitoring, referring, and linking with other providers. 
  • Develop, update, and monitor annual paperwork for individuals receiving psychiatric services (Individual Plan of Service, necessary assessments, and other paperwork requirements). 

  • Collaborate with prescribing providers to align services with clients’ needs and treatment goals. 

  • Ensure service plans comply with agency policies, Medicaid guidelines, and other regulatory requirements. 

  • Facilitate coordination between psychiatric and medical services to address both mental and physical health needs, when necessary. 

  • Assist clients in identifying and establishing a primary care provider (PCP) for non-psychiatric medical needs. 

  • Educate clients on the importance of integrated healthcare, including medication adherence and preventative care. 

  • Advocate for client needs within the healthcare system, ensuring they receive appropriate and timely services. 

  • Document all case management activities in accordance with agency policies and confidentiality guidelines. 

  • Maintain up-to-date clinical records and service documentation in the electronic health record (EHR) system. 

  • Ensure compliance with HIPAA, Medicaid, and agency policies regarding client confidentiality and service provision. 

  • Track client engagement and follow up on missed appointments, care coordination needs, or changes in mental health status. 

  • Perform other duties as assigned.
